        Student Health

A full time registered nurse is assigned to Wales Elementary School and may be reached

between the hours of 8:15 and 3:00 at the school.

          School Health Requirements

Under Massachusetts State Law, no child will be admitted to school without the
required immunizations. Children entering kindergarten are required to have a full
physical prior to the start of school. Fourth grade students are also required to
have a physical exam. Once a year hearing and vision screening is conducted for the
children in grades K through 6. Postural screening is conducted for all the fifth and
sixth grade students. In the case of concerns, parents will be notified to consult
their Physicians for further evaluation.

In case of illness or injury, emergency care and first aid will be provided until a
parent or other assigned adult responsible for the child is contacted. Please make
certain that we have an updated emergency procedure card for each of your children.
Other special issues, such as legal custody and allergies should be noted on the card.
Please keep the school posted on any address or telephone number changes as, in the
event of an emergency, we would need to reach you in a timely manner. Parents must
provide transportation home for any sick or injured child, except in an emergency.

Children may NOT bring any medication, prescription or non-prescription drugs
(i.e. inhalers, Tylenol, Advil, lactaid medication or cough drops) to school.

If medication is necessary, an adult must bring it in. We can only allow medication
in the original prescription container and it must be accompanied by a medication
form signed by the doctor and the parent/guardian. The school nurse will administer
all medication. The parent/guardian has the option to accompany the child during
field trips to administer the medication on those days. Antibiotics can be scheduled
around school hours. A child on antibiotics for a communicable disease must remain
out of school for at least 24, preferably 48 hours after the initial dose.

We ask that a parent/guardian please notify the school by 9am if their child is going
to be absent or tardy. Your phone call assures us that your child is safe.

Thank you for your cooperation in these matters.
We look forward to sharing a fun,
healthy and exciting school year with your child!
