| Mrs. Brogan, R.N., is the school nurse at the Junior High. She
is mandated by law to keep accurate health records and to inform students
and staff whenever physicals or specific shots are necessary. Her office
is located on the first floor near the Guidance Office.
The following forms may be printed for completion
by private physicians for the parents. Please print in small or
medium text view. Private Physician's Examination Form for Vision, Hearing, or Postural Screening Private Physician's Physical Examination Form General Information Physical Exams - A physical, documented on a form and signed by the doctor, is required to participate in the after-school sports program, and for all entering 7th graders. Sport exams for athletes should be completed annually, preferably after June 1st. Exams remain current through the chosen sport season, and will be on file in the health office prior to participation. Please be aware that the school sponsored sport exams are scheduled for the first week of school in September and the beginning of November, March, and June. If the student's physical exam is outdated or not on file with the nurse, the student may not try out or participate. Form available. Immunizations - The health records of all entering 7th grade students will be reviewed for booster shots. Entering 7th grade students are required to have had an MMR#2, a TD booster (if 5 years since last DTP), Hepatitis B 3 doses and Varivax 1 dose (2 doses for 13-years or older), or a doctor's certificate indicating a positive history of having had Chickenpox. Medications - No medications, including non-prescriptive medications, are to be at school. Only those medications prescribed by a physician and accompanied by the Parent Permission - Doctor Order Form and kept in the original container will be given by authorized school personnel. Short term medication given three or four times a day should be handled at home. Transportation of medication will be arranged by the nurse and the parent. Daily medications will not be given on early-release days nor given on field trips, unless specifically requested in writing by the parent. When there is a delayed opening to school, please remember to give the home medication at the usual time to avoid confusion at school. If your son or daughter needs medication at school for any of the following reasons: asthma, bee sting allergy, or diabetes, please speak to the nurse personally once school has re-opened. Form available. Injury/Illness - Injury/illness acquired at home may affect the student's education. Parents are asked to contact the nurse if their sons/daughters will be absent for an extended period of time. Students who have sustained an injury at home should bring in a parent note to the nurse stating the nature of the injury and possible school restrictions. Students who are ill should be kept at home. Those needing to be dismissed from school for medical reasons must do so through the health office. Students who are absent five days or longer will need a doctor's certificate to re-enter school. Screening - State health regulations require that vision, hearing, postural screening, height and weight be done annually for all students. Form available. Medical Gym Excuses - Students are permitted two parent-written excuses per quarter. Gym excuses lasting into a second week or longer will require a doctor's note. Face/ear/body jewelry will be removed prior to participating in gym class. Protection of all students from potential injury, where bleeding may occur, is paramount in order to avoid any exposure to Hepatitis B and HIV. Hints for Healthy Living
