Procedure:

The following is needed to register a child for school.  You may pick up a registration packet at the front office during school hours.

1. Emergency Information Card
2. Letter of Discipline status from previous school
3. Massachusetts School Health Record;  Immunizations from previous school
4. Proof of Residency (2 documents)
5. Publicity Release Form
6. Registration Form
7. Request for Records
8. Student Data Sheets
9. Transfer Card from previous school

Your child can be admitted to school only when all of these documents are completed.
