Effective March 19, 2004 the process for issuing work permits to 14 and 15-year-olds changed.

(promise of employment and physicians certificate of health)

This application contains the listings of restrictions applicable to this age group.   Once completed, the form is brought to the main office of either the high school or junior high school where the actual Employment Permit will be completed by the office staff.   Both forms are to be brought to the Superintendent’s Office for authorized signature.

The process for students 16 and 17 years old has not changed, although the form is different.   Students should go to the main office of the high school to have the form filled out and bring the completed form to the Superintendent’s Office for authorized signature.
